High Intensity Care

NDIS Medical supports

We understand the importance of managing your personal care at home.

Our High-Intensity Care service is designed to support people with complex health, medical, behavioral, or aging needs.

Our respectful and well-trained staff are dedicated to assisting you with caring and compassion, with the aim to provide the best assistance in the comfort of your home to live a life with independence.

We offer the following services:

  • Complex care management including bowel, ventilation, urinary catheter, diabetes and more.)

  • Use of specialised aids and appliances including hearing and communication devices

  • Implementation of all Allied Health Programs

  • Dedicated Registered Nurse providing clinical oversight.


Supported Independent Living (SIL)

Transitioning to independent living is an exciting step, but it can also be a bit scary.

You might need a little help now and then with personal care or medication, or maybe you need someone with you full time to help with those things. Our team will tailor a plan to get you there whether that means occasional support to attend appointments, having someone with you full time to help with personal care and medication, or anything in between.

We offer the following services:

  • Implementation of Allied Health Programs that have been developed for you

  • Assistance with daily personal activities such as bathing, getting in and out of bed, and other needs

  • Assistance with daily household tasks such as cleaning, home maintenance, and other services

  • Assistance with other daily tasks such as taking medication, attending appointments, and shopping

  • Providing groceries and meal planning that are reflective of your culture and religion.


Specialist Disability Accommodation (SDA)

At Transform NDIS Supports, we provide comfortable and safe Specialist Disability Accommodations (SDA) to help promote the independence of those with a disability.

We have worked on our own new builds and partnerships with other stakeholders to establish homes that meet individual needs.

Additionally, our SDA service also provides assistance to help you through the difficult process of finding suitable accommodation and supporting you through the required legal and administrative documents while ensuring your interests are protected.

Our Specialist Disability Accommodations are suitable for participants who have:

  • Intellectual disabilities

  • Difficulties with mobility

  • Extreme functional limitations

  • A high level of support requirements

  • A reasonable need for specialist disability accommodation.


Psychology Recovery Coach

Transform NDIS Supports has developed a unique Psychosocial Recovery Coach (or NDIS Recovery Coach) services. This is our new mental health-focused NDIS support program that is designed for participants living with a psychosocial disability to manage complex challenges in their daily living.

This support is programmed to maintain engagement through periods of increased support due to the episodic nature of the mental illness. Our coaches work collaboratively with participants, families, carers and other services to identify, plan, design and coordinate NDIS supports.

We offer:

  • Developing a recovery-enabling relationship

  • Supporting participants’ engagement with the NDIS

  • Coaching therapy to increase recovery skills and personal capacity

  • Supporting participants with their recovery planning

  • Collaborating with the broader system of support.


Mental Health Building Capacity

Working with community mental health teams across Adelaide, our community mental health programs are embedded with comprehensive risk and care plans incorporating all allied health providers information and implementation towards capacity-building aimed at building personal skills and fostering greater independence. We focus on empowering individuals to build confidence while closing the loop.


Community Engagement

Looking to get involved more with your community or just looking to learn new skills and try a new hobby? Then this is the right program for you.

In this support service, we assist with activities such as using public transport, attending community events, joining a gym or play sport, visiting a local library, going to the movies or music concerts and participating in volunteer groups. We can also help you to maintain existing friendships and make new ones.


Positive Behaviour Support Engagement

In collaboration with therapists our Positive Behaviour Support Engagement implementation includes restrictive practice monitoring, reporting and reduction.
